What is Form DS-11?

Form DS-11, Application for a U.S. Passport, is the U.S. Department of State form used to apply for a passport book or passport card in person. It is the application first-time applicants and several other groups must use.

Because DS-11 is submitted in person, you fill it out and print it, then present it with your documents at an acceptance facility. You do not file it online. The template loaded here is the official current edition.

What is Form DS-11 used for?

DS-11 is the in-person application for a new U.S. passport. You use it to:

• Apply for your first U.S. passport book or card.
• Replace a passport that was lost, stolen, or damaged.
• Apply for a child under age 16.
• Apply when your most recent passport was issued before you turned 16 or was issued more than 15 years ago.

Who needs to fill one out?

You must apply in person on Form DS-11 if any of these is true:

• You are applying for your first U.S. passport.
• You are under age 16, or applying on behalf of a child under 16.
• Your previous passport was lost, stolen, or damaged.
• Your previous passport was issued when you were under 16.
• Your previous passport was issued more than 15 years ago.
• Your previous passport was issued in a previous name and you cannot document the name change.

If you can renew by mail, you would generally use Form DS-82 instead of DS-11.

What information is required?

Before you start, have the following ready:

• Your full name and any previous names you have used.
• Your date and place of birth.
• Your Social Security Number.
• Your mailing address, email, and phone number.
• Basic details such as height, hair color, eye color, and occupation.
• Emergency contact and, if relevant, travel plans.

What documents do you need?

Bring these to your appointment along with the printed form:

• Proof of U.S. citizenship, such as a certified birth certificate or a naturalization certificate.
• A photocopy of that citizenship document.
• A valid photo ID, such as a driver's license, plus a photocopy of it.
• One passport photo that meets the State Department requirements.
• Payment for the applicable fees.

How to fill out Form DS-11

Open the form above and work through it section by section:

• Choose whether you want a passport book, card, or both.
• Enter your name, date of birth, place of birth, and Social Security Number.
• Add your mailing address, email, and phone number.
• Complete the physical description and parental information as requested.
• List any prior passport details and complete the additional pages if they apply.
• Leave the signature blank. You sign in person when the acceptance agent asks.

What are the fees?

A DS-11 application has two parts: the passport fee paid to the U.S. Department of State and a separate acceptance fee paid to the facility where you apply. Amounts depend on whether you choose a book, a card, or both, and on optional expedited service. Check travel.state.gov for the current fee amounts before your appointment.

When DS-11 is not the right form

You do not use DS-11 if you are eligible to renew by mail, which most adults can do when their most recent passport is undamaged, was issued in the last 15 years, and was issued when they were 16 or older. In that case you use Form DS-82. There are also separate forms for correcting or changing the name on a recently issued passport.

Where to submit it

Submit DS-11 in person at a passport acceptance facility, such as many post offices, libraries, and clerk of court offices, or at a passport agency by appointment. Bring the printed form, your documents, your photo, and payment, and sign the form there in front of the acceptance agent.
